We are excited to share that CMS has finally released the Next Generation ACO results from performance year 2019! Trinity Health ACO achieved a 95% quality performance score and $24.4 million in shared savings — it’s highest performance in its four years of participation.

As Affinia Health, we participate as the West Michigan Chapter of the Trinity Health Accountable Care Organization (ACO) (THACO), one of Medicare’s Next Generation ACO (NGACO) programs. Our partners in this contract with the Centers for Medicare and Medicaid Services (CMS) include Trinity Health chapters in Columbus, Ohio, Loyola in Chicago, and Summit, an independent medical group in New Jersey. We function as a unit and are evaluated by CMS as a unit. Affinia Health Network has entered into our 6th year of participating in the Trinity Health Accountable Care Organization. A CMS requirement of this program is that all beneficiaries receive a letter from Trinity Health Accountable Care Organization informing them that they are a beneficiary in the program through their provider. This letter will be mailed to patients on April 26, 2021. The letter provides a phone number the beneficiaries will call for any questions they may have.
