  • Utilize simple finger stick methods of blood collection & provide immediate results that are managed by the pharmacist at the time of visit
  • Improve safety by providing extensive patient education during appointments
  • Provide written instruction to the patient at each visit including dosing schedule, next appointment, and a list of current medications

What are the benefits of pharmacist provided anticoagulation management?

The pharmacist performs the following activities when managing anticoagulation therapy:

  • Assistance in choosing or verifying that the most appropriate anticoagulant is ordered
  • Verifying appropriate dose of anticoagulant is selected based on specific patient characteristics
  • Providing direction of when to hold medication based on to lab results or scheduled procedures that may require bridging
  • Monitoring adherence to therapy
  • Monitoring kidney and liver function
  • Sending refills to the outpatient pharmacy
  • Providing education to the patient related to anticoagulation therapy & management
